Attention students in ED 4799 The Professional Teacher:  

When submitting student teaching requests, forms must be typed and assembled (stapled) in the following order:

• 3 packets containing (top to bottom): student teaching request form; cover letter; resume; information form.
• 1 packet containing (top to bottom):  cover letter; resume; information form.

Your signature must be on every cover letter.
