APPLICATION PROCESS FOR WORK-BASED LEARNING ENDORSEMENT:

Once all five courses have been completed, pull the form; Application for MN Education License (Teaching/Administrative/Related Services) from the Minnesota Department of Education site http://education.state.mn.us/html/intro_licensure_forms.htm . Complete as an Addition of a new field to a valid Professional License.

Complete pages 1,2,4, & 5. Send the completed form and a $57.00 check made out to Minnesota Commissioner of Education to: Kathy Jackson, Bemidji State University, 1500 Birchmont Dr., Box 34, Bemidji, MN 56601.  BSU will complete part 5 of the application and forward the complete application to the Minnesota Department of Education.
