Course Work Only

If you are admitted as an on campus student and wish to register for an online or guided/self directed course for Fall and Spring semesters, you will need to secure permission from your advisor, the course instructor, the department chair and the college dean.  Please print off this Course Work Only form complete the requested information, and secure the signatures.  The Center for Extended Learning staff will register you for the course when we receive the signed form.  NOTE:  Advisor, instructor, chair and dean signatures are NOT needed for summer courses.

Please note: 

• If you have senior status (96 credits or more) you will only need the signatures if you are requesting registration for a BUAD, PHED or HLTH course. 
• Freshman, sophomores, and juniors will not be given permission to register for online BUAD courses.
• CEL credits (online and guided/self directed) are not included in the tuition band.
• NO signatures are required for SUMMER online or guided/self directed courses beyond the student's signature.
