Application for Admission to the School of Graduate Studies as a regular admit into a specific graduate program, a graduate special student, and/or FasTrack (Secondary Education Initiative).

Candidacy (Form 2)

The purpose of the application for candidacy is to insure the student is a qualified candidate for a Master's degree program and to set forth those courses to be included in the program of study.

M.A./M.S. Track (Forms 3, 4, & 5)

For those pursuing a Master of Arts or Master of Science degree.

M.Ed. Track (Forms 3, 4, & 5)

For those pursuing a Master of Education degree.

Graduate Petition (Form 6)

For the purpose of making changes to your approved candidacy form such as substitutions, time extension, etc.
