Human Subjects Approval

Human Subjects approval is for the purpose of securing approval to administer surveys, interviews, etc. to human subjects. Approval must be secured before the survey/interview is administered. All Human Subjects approval forms and attachments must be typed.

The BSU Human Subjects Committee has mandated that any BSU student, faculty, or staff who will conduct human research, will need to complete and include in the Human Subjects proposal packet a Certificate of Completion of the National Institutes of Health (NIH) Office of Extramural Research Web-based training course “Protecting Human Research Participants”.  The student will only need to take the training once while attending BSU. All BSU faculty and staff who act as a Sponsor/Advisor must also provide a certificate of completion; the faculty/staff will need to re-take this training every five (5) years.

• Should you have changes to your protocol, consent forms, or any supportive materials (such as advertisements, questionnaires, surveys, etc.) AFTER approval has been given on your initial proposal, please complete the Protocol Change Form, secure the appropriate signature(s), and submit along with revised research documents. All changes must be highlighted.
