Forms

Students and Teachers will both need to submit forms.

Students

Students need to submit two different documents or sets of documents, in order to register for the fair.

First, each student needs to register for the regional fair.

This registration form requires Adobe Acrobat Reader which can be downloaded for free.
Notice: Mac users must also use Adobe Acrobat Reader , because Preview.app will not properly submit the registration.

After you complete the form, please Save it.  Then try to "Submit" the form. 

If submitting the form results in an email window opening up, send the email, and you should be done. 

If submitting the form does not result in an email window opening up, open your email program, and attach the registration form that you saved, and send an email to:ScienceFair@bemidjistate.edu

Second, each student needs to complete the proper International Science and Engineering Fair paperwork (ISEF). This needs to be received by Feb. 14th, 2012. One of the best ways to determine the paperwork needed is to use the ISEF forms wizard. This will give you a list of forms that you need to complete, based on the type of project you have. These documents, along with a check for registration fees and the cost for an optional lunch should be sent to:

Check with your teacher before sending in fees or forms, as they may wish to send all the forms for your school in one packet.

Teachers

Please submit the Teacher Summary Form:

This allows us to track all participants from a school.  The form can be sent with the ISEF packets from students, or can be emailed as an attachment to ScienceFair@bemidjistate.edu
