Curriculum Development Forms

The following links will take you directly to the official forms needed for making curriculum changes.  All requests for changes require Forms 1 & 8.  Other forms are necessary depending on the nature of the change.  Please read the instructions carefully.  The checklist provided will make your movement through the curriculum change process smoother.  It is also strongly advised that you have one departmental editor for all forms, and that you receive editorial assistance from Kyle Crocker, Curriculum editorial assistant,  kcrocker@bemidjistate.edu  prior to submitting your proposals for approval.
