Faculty and Course Evaluations

EXAMPLE SURVEYS (from eLearning Support at BSU)

eLearning Support has created example surveys for faculty and course evaluations. You can click on the links to view the example surveys: Student Evaluation of Faculty (PDF) ... Student Evaluation of Course (PDF)

Either or both of these surveys can be copied into your D2L class shell. You can then revise or delete any of the questions.

REQUEST for COPYING: To have one or both surveys copied into a D2L class shell, please contact Julie Adams. The surveys will be inactive.

To make a survey active:

  1. In the D2L class, click on Surveys in the navigation bar.
  2. Click on the name of the survey.
  3. Click on the Restrictions tab.
  4. Under the heading "Availability," use the dropdown for Status to change "Inactive" to "Active."
  5. Click on the Save Survey button.

NOTE: You will probably want to post a News item in D2L for your class to let the students know that there is a survey that you want them to respond to. You should let the students know that the survey is anonymous.

Click on the link to view a sample classroom assessment survey for use in D2L. You can either create your own survey in a D2L class shell using the questions in the example, or you can contact Julie Adams to have the survey copied into a D2L class shell.

Quality Matters (QM) is a faculty-centered, peer review process created at the University of Maryland and sponsored by MarylandOnline, Inc. Quality Matters is nationally recognized for its peer-based approach to quality assurance and continuous improvement in online education and course design.
