Distance Delivery Application Form

The BSU Library will deliver materials to distance learners (students who are currently registered for BSU courses and current BSU faculty) who reside 30 or more miles from campus or  who  need delivery because of permanent or temporary disability.  Non-returnable items (journal articles, book chapters, etc.)  are delivered electronically to distance learners. Returnable items (books, videos, DVDs, etc.) are delivered via tracked mail (with delivery confirmation) at the BSU Library’s expense and returned to the BSU Library at the expense of the distance learner. All interlibrary loan overdue policies apply for returnable items.

By submitting this form, I request delivery of returnable interlibrary loan materials and BSU Library materials to my home address. I authorize BSU Library staff to check out materials in my name, and I also understand that I am responsible for the loss of any materials returned to the library using a shipping service without package tracking and/or insurance. I understand that non-returnable items (journal articles, book chapters, etc.) will be provided to me electronically, to the BSU e-mail address listed above, or mailed to my home address if e-mail delivery fails.
