All students enrolled for credit instruction and who are assessed the student fee are eligible to use the Health Service. Proof of student fee payment is required prior to service. A properly completed medical history form is essential to the Health Service staff in providing appropriate treatment and is required of all students prior to treatment. Failure to comply with either of these requirements may result in denial of service.
Students with health conditions that unreasonably endanger the safety and welfare of themselves and/or other members of the University community may be removed from the University and University facilities, have their enrollment canceled or be denied enrollment at the beginning of a semester if their condition persists. After considering available information, the Medical Director of the Student Health Service, the Office of the Vice President for Student Development & Enrollment and the students' Academic Dean shall determine jointly whether these students shall have their current enrollment canceled or will be permitted to register.
