Application to request an American Sign Language Interpreter for BSU Events

Applicants requesting an ASL interpreter for campus events should fill out the appropriate side of the form and submit it to the Disability Services Office a minimum of two weeks prior to the event.  Please sure all contact information is included so we can notify you of the person filling your request and where your interpreter will meet with you.  See application form above.

Students needing an ASL interpreter for courses and academic projects should bring this form to the office of the Coordinator of Disability Services with the appropriate information once you have completed your registration on priority registration day.  A copy of the syllabus for the course should be submitted with the request whenever possible.  See application form above.
