If an accident or injury has occurred that involves a BSU employee, it must be reported to a supervisor and Human Resources.

This reporting method is for non-emergency incidents only.

Please give us as much information as you are willing or comfortable providing so that we can adequately address the issue.

Please note that EHS will evaluate your report and share details with relevant parties as needed. Details of the incident may be shared, however we will make every effort to protect your personal information. Please remember that all information contained within your report will become of the EHS administrative record keeping.

Depending on the nature of the incident and the details provided, EHS may or may not address the issue directly and we cannot guarantee a response to every reported safety concern.

To report a safety concern, please email and include the following information:

  • Location where the incident occurred (required)*
  • Date and time
  • Description of unsafe condition or incident (required)*
  • Ideas for corrective action (optional)
  • How we can contact you if we need to follow up (optional)
