{"id":123,"date":"2022-01-20T17:42:23","date_gmt":"2022-01-20T23:42:23","guid":{"rendered":"https:\/\/www.bemidjistate.edu\/offices\/environmental-health-safety\/?page_id=123"},"modified":"2023-01-09T15:53:07","modified_gmt":"2023-01-09T21:53:07","slug":"report-an-injury-or-accident","status":"publish","type":"page","link":"https:\/\/www.bemidjistate.edu\/offices\/environmental-health-safety\/report-an-injury-or-safety-concern\/report-an-injury-or-accident\/","title":{"rendered":"Report an Injury or Accident"},"content":{"rendered":"<p class=\"intro\">Injuries to any BSU\/NTC employee, student or visitor to the campus need to be reported to the appropriate office as soon as possible.<\/p>\n<h2>Emergencies<\/h2>\n<ul type=\"disc\">\n<li>Call 9-911 immediately for life threatening or serious injuries.<\/li>\n<li>Do not delay seeking emergency medical attention in order to report an injury or complete forms.<\/li>\n<\/ul>\n<h2>Non-Employees &ndash; Students, Visitors<\/h2>\n<ul>\n<li>Report the injury to BSU Public Safety, (218) 755-3888, as soon as possible.<\/li>\n<li>Complete the&nbsp;<a href=\"https:\/\/www.bemidjistate.edu\/offices\/environmental-health-safety\/wp-content\/uploads\/sites\/89\/2022\/01\/non-employee_injury_report_form.doc\">Non-Employee Injury Report Form<\/a>.<\/li>\n<li>Submit the completed form to:\n<ul>\n<li>BSU Business Services<br>\nDeputy 201, #5<br>\nPhone: (218) 755-2052<br>\nFax: (218) 755-2160<\/li>\n<\/ul>\n<\/li>\n<\/ul>\n<h2>Employees &ndash; Faculty, Adjunct Faculty, Staff, Student Employees, Graduate Assistants<\/h2>\n<ul type=\"disc\">\n<li>Report work related injuries or illnesses to your dean or supervisor as soon as possible. If your supervisor is not available, contact the Human Resources Office at (218) 755-3966 or Environmental &amp; Health Safety at (218) 755-2545.<\/li>\n<li>Assist your supervisor in the prompt and accurate completion of reporting forms required for the Workers&rsquo; Compensation program.<\/li>\n<li>More detailed information about the <a href=\"https:\/\/www.bemidjistate.edu\/offices\/environmental-health-safety\/workers-compensation\/\">Workers&rsquo; Compensation program<\/a> is available.<\/li>\n<\/ul>\n<h2>Supervisors<\/h2>\n<ul type=\"disc\">\n<li>Download the&nbsp;<a href=\"https:\/\/www.bemidjistate.edu\/offices\/environmental-health-safety\/wp-content\/uploads\/sites\/89\/2022\/01\/Supervisor-Checklist-BSU-rev-12-15-14.pdf\" target=\"_blank\" rel=\"noopener\">Supervisor&rsquo;s Injury\/Illness\/Incident Reporting &amp; Workers&rsquo; Compensation Checklist<\/a>.<\/li>\n<li>Complete all the items listed in the Supervisor&rsquo;s Checklist.<\/li>\n<li>Submit the forms to the&nbsp;<a href=\"https:\/\/www.bemidjistate.edu\/offices\/human-resources\/\">BSU Human Resources Office<\/a> as soon as possible, but no later than three days after being informed of an employee&rsquo;s work-related injury.<\/li>\n<\/ul>\n","protected":false},"excerpt":{"rendered":"<p>Injuries to any BSU\/NTC employee, student or visitor to the campus need to be reported to the appropriate office as soon as possible.
