The <a href=\"https:\/\/www.bemidjistate.edu\/admissions\/undergraduate\/accepted-students\/immunization\/\">BSU Immunization Form<\/a>\u00a0is designed to provide the school with the information required by the law and will be available for review by the Minnesota Department of Health and the local health agency. <strong>Be sure to bring your immunization records with you when you first arrive in Bemidji.
