Update Your Program

Do you need to update your program?

If you are a graduate student who meets one of the following criteria, you must update your program.

  • Application Addendum
    Students must use this form if they meet one of the following criteria:
    • Student wishes to change a program choice on a currently active application
    • Student wishes to add to a current program (i.e. a license, endorsement, certificate, or additional graduate degree)
    • Complete Form

Please note: Programs may require additional application materials.  Visit the Graduate Application Dashboard to check your application status and submit additional materials.

    • Student wishes to return to their previous program and graduate status only after having discontinued course work for a period longer than two years
    • Complete Form

Please note: Students who wish to begin an entirely different program or status must reapply. Contact the School of Graduate Studies if you have issues reapplying.

  • Reapplication
    Students must use this form if they meet one of the following criteria:
    • Student wishes to complete an application after canceling/discontinuing an application or admission status over a period longer than one year
    • Student wishes to pursue a graduate program after withdrawing from either the same or different program
    • Student wishes to pursue a different program after completing a previous program
    • Visit Graduate Admissions

  • Graduate Studies Withdrawal Form
    Students must use this form if they meet one of the following criteria:
    • Student wishes to cancel a current application
    • Student wishes to withdraw from a program or the School of Graduate Studies or Bemidji State University
    • Complete Form
