Students completing an Applied Masters will submit this form during their final course.

Applied Masters Application for Graduation - Form 4

Submit the following information to the School of Graduate Studies.
  • Degree Information

  • Please read and acknowledge the following Applied Master's information:

    Before a student submits Form 4, Application for Graduation and preliminary curriculum vita (CV) for an Applied Masters, the student must first have the following items approved and on file in the School of Graduate Studies: a) Been officially admitted into an applied master’s program through the application process; b) Form 2-Application for Admission to Candidacy (grad plan); c) Form 3-Graduate Project and Presentation Proposal Approval; and d) have completed all course work (core requirements and electives) prior to applying for graduation. Form 4 should be submitted to the School of Graduate Studies when the student registers for the final course requirement.

    After the Form 4 and preliminary curriculum vita (CV) have been submitted, students are strongly encouraged to work closely with their Advisor/Final Course Instructor to complete their final project and presentation.
  • Submission of Curriculum Vita

    A preliminary curriculum vita (CV) should be submitted along with your Form 4-Applied Master's Application for Graduation. Instructions and templates for the Curriculum Vita may be found in the Applied Masters track under "Current or Returning Students." WARNING: If you have not yet completed your curriculum vita, do so now and submit along with your Form 4. You may, however, complete the CV as a Word document and submit to as an email attachment.
